English 2 – Literary Response

My Singular Irene Literature is, by my understanding, a means to critic society and individuals or the way the world is behaving. Writers of Literature write literature to provide a new way of seeing things that might be different from our point of view. In other words, writers use tools of literature story like: symbolism, point of view to help them is making the literature story to be more exciting and clear to us, the reader. The author helps us to see the frailty or other bad deeds and good things about this life in this world as we seldom see those things with our own eyes. A Very Old Man With Enormous Wings and My Singular Irene is a very good example for this literature reading, because it is very rich in literature concept and devices, like: symbolism or point of view. The main character in this story is said to be alienated, repressed and silenced in the story. I will prove this statement by retelling the main point on the story that supports this statement. A very old man refers to an old angel that is cast ashore into earth because of inability to keep up with the heavy rain. e can see that the angel is alienated from examples that shown this statement, such as : †They found the whole neighborhood in front of the chicken coop having fun with the angel, without the slightest reference, tossing him things to eat through the openings in the wire as if he weren’t a supernatural creature but a circus animal. †(Gabriel Garcia Marquez 36). the angel is alienated because It is treated differently from the others. The others are having f un with the angel rather than socializing with the angel. The angel is also repressed and silenced. There is a quotation that shows this statement: â€Å"Pelayo watch over him all afternoon from the kitchen, armed with his Bailiff’s club, and before going to bed he dragged him out of the mud and locked him up with the hens in the wire chicken coop. †(Gabriel Garcia- Marquez 36). From this statement, we can derive that the angel is â€Å"caged† from it’s freedom and its freedom is repressed by the caged that Pelayo given it, thus the angel is silenced from the freedom. My Singular Irene is also a good example for this literature reading. Irene is reputed to be alienated is true in this story. Irene is treated differently from other people. From the story we know that Ilene’s husband( the first person view in the story) isolated Irene from the outside world by buying Irene very expensive â€Å"stuff† to lure Irene to stay in the house isolated from the outside world. Irene is also axiomatic to be repressed and silenced. We know from the story that Irene is considered as a â€Å"thing† to her husband, because her husband wants Irene to act like he wants. So, in order to satisfy his ego, Irene is inhibited and muted by many ways, like: not allowing Irene to go to the outside world, forbidding visitors to meet Irene. Irene too, like the Angel is constrained from freedom. A Very Old Man With Enormous Wings and My Singular Irene are an example of story that has a symbol of contemporary alienation and triumph over it. First, I am going to express about A Very Old Man With Enormous Wings. The angel in the story is alienated or make indifferent. It is derived from the behavior of the people that found the angel. It is treated like mere goods than divine figure. So, it is true that it is a contemporary alienation. Later in the story, we know that the angel has his most precious tool that can bring him back to his divine status. That tool is its wings, when its wings grows back, he immediately leaped up to the sky and clapped its wings and fly to the horizon of freedom. The angel is reputed to be a triumph over alienation. Second, My Singular Irene is also a symbol of alienation. Irene is also made hostile like the angel. She is isolated or â€Å"caged† by her husband; there is an interesting point in this story in my point of view. To me, Irene seemed to be speechless or not a living thing. The author writes the story in such a way that us, readers, grabs the alienation theme in the story. Irene is also considered as a triumph over alienation. There is a quotation from the story: Horrifying! Irene changed into a horrendous butterfly! I stood up and fell again, powerless now. She would leave me; she would take flight and leave me. The gigantic Irene Butterfly smiled at me, diminished, and disappeared with the others. I am at the site of the events, waiting for Irene’s return. She has no return. She cannot deny me the peace that her company always offered me. (AlmAnzar, JosE AlcAntara 45) From this quotation, I interpret it that Irene’s freedom does also mean she has a higher status in the relationship with her husband now. What I meant by higher status of relationship is the one that always try to get the relationship going is the one that has a lower status, in this case, her husband. Irene’s triumph over alienation is expressed by the author of Irene transformation into the most beautiful butterfly that can fly anywhere it wants. It is very interesting that these two stories have a similar thing. They both define freedom by flying leaving the thing that binds their way to freedom. The following paragraph below tells a story about human needs for personal freedom and community. There is certainly no argument in this world that can be used to defy human needs for socializing and personal freedom. Human always seek out a way to grab that freedom. Kingdom falls, kingdom rises, Leader of a faction dies, and assassin was born. All of these examples are made because of the need to obtain and maintain personal freedom and the ability to communicate freely with communities of choice. There is an interesting story below those talks about human’s desire for personal liberty. This story is about the struggle of African-American communities in the United States to obtain their right of personal freedom and human needs for sociality. United States also known as the land of the free and the brave has been well known for its freedom. It has a lot to offer, such as: the freedom of speech, the right to work, etc. But, in contrast to all of those things, there is a little dispute inside this great country in the era of slavery. African American is the target for this matter. African American arrived in the United States as Indentured Servants via Jamestown, Virginia in 1619. They could earn their so called â€Å"personal freedom† by working as slave to their chosen master. As time goes by, the other majority in the United States made a conduct for slaves called â€Å"Slaves Codes†. This Slave Codes constrained African American in the Unites States from their liberty. They did fight for it until the Majority pushes another harsh rule for any slaves that disobey its masters. From this point, we can see human’s needs for personal freedom. Human fights for freedom risking everything they had, like: their lives, treasure, relatives. There is also a story of a president that fight for part of his citizen’s needs for personal freedom, his name was Abraham Lincoln. He was a farmer boy that born Feb. 12, 1809, in Hardin County, Kentucky. His parents were both born in Virginia. Abraham was well known for his strength and cleverness in farming skills. In not so long time he began to be very famous in the community because of his skills in socializing. Not so long after that, he became the president of the United States. He saw this great calamity of slavery and intends to abolish this calamity for good. To pursue this personal freedom, Abraham had gone through a lot of obstacles from his own family until his own citizen in the south. Alas, not so long a civil war broke out between the north and the south. But, the good always wins; Abraham with the North wins the Civil War by diplomacy. And thus the period of reconstruction began. Unfortunately Abraham was killed by an assassin in April 14, 1865; Lincoln was killed at Ford's Theatre in Washington by John Wilkes Booth. This story has greatly described to us the importance of human need for community and personal freedom. In conclusion, these two stories, My Singular Irene and A Very Old Man With Enormous Wings, tell us about the importance of personal freedom and need of interaction with other people. They also told us, no matter how hard human is pushed to abandoned their personal freedom; human always emerges to fight their way to obtain freedom even though they have to put their lives on the line to achieve their goal, personal freedom.

The counselor’s thoughts and feelings in reaction to the client can be from the client themselves or events in the counselor’s own life. Counselors should monitor their feelings while working with their clients.Counselors who are aware about their personal issues allow them to manage countertransference. They can feel empathy for their clients and compassion which is helpful in the therapeutic process. Therapists who manages countertransference recognizes their reactions and how it affects the therapy process. A counselor’s unresolved issue is the basis of countertransference. When a client talk about issues related to a therapist’s unresolved issue, the therapist te nds to avoid the issue, according to Rosenberger and Hayes article.They will talk about the issue less in sessions with the client. Counselors dealing with unresolved issues pertaining to anger are more comfortable and anxious with angry clients. Therapists showing avoidance behaviors can cause clients to view them as being incompetent and untrustworthy. I agree with the findings in this article. Therapist avoidance also effects the working alliance, the therapist’s social influence, and the sessions. The article discusses what happens when a client brings up issues that are related to a therapist’s unresolved conflict.Managed countertransference results in improvement in insight, self-awareness, and resolution in therapy. A counselor’s ability to respond with empathy, genuineness, respect, acceptance, and caring influences the process of helping the client to identify their problems. Counselors must be able to pay attention to their clients. They must be able t o set aside their own problems in order to work with client issues. Counselors in need of resolving unresolved conflicts may want to attempt to receive counseling themselves.

When a company is considered a poor financial risk, the public will not be in a hurry to buy its stock. So who is affected by finance? Shareholders, as mentioned previously, are the focus in publicly traded companies. They are not the only people who think about financials, however. The CEO, CFO and any other "C" position have accountability to report to the board about the financial performance of the company. Management is responsible for creating and maintaining both capital and operational budgets. Employees are required to maintain certain standards of productivity. Customers are affected by finances as well. Consider gas prices, and how increased costs in production are passed on to the consumer. When looking at a company's finances, there are essentially four items to consider: the income statement, the price earnings ratio, the balance sheet, and the statement of cash flows, (Block, 2005). The income statement is a tool used to measure profitability over a given period of time, i.e. quarterly, annual. The income statement evaluates the cost of producing goods or services and the money that was made as a result of selling those goods/services. Gross profit and net earnings are two key features to look at. The price earnings ratio measures the relative valuation of earnings, (Block, 2005). This is a way of looking at how your company's stock earnings compare to other companies both within and outside your industry. This ratio is affected by many variables like marketability, sales growth, and the debt-equity structure of a company.

We are thrust into a ride, much like one you might find at the amusement park or the carnival, with the distinct difference that although this ride is in fact on rails, how it will affect us and how we will interpret the events during is completely up to us. Edgar Allan Poe does a remarkable job of employing several psychological techniques in his short story The cask of Amontillado , but I will only focus on one, which even by todays standards is flawless. The technique is the mystery. Who is Fortunato? What has he done to Montresor that has caused so much emotional and psychological damage? Obviously the answers to these questions will elude and intrigue the audience. So we are instantly on the hook. To find the answers to these questions we must avert more of our attention and interest to the piece at hand. Poe, now with our utmost and full attention, begins to plunge us into the mind of his protagonist. Not so by simply introducing us to Montresor but instead by showing us his actions, his thoughts, his mannerisms. He accomplishes this by exposing us to Fortunato and the conversations between them that will ensue. On the surface Montresor seems like a normal man with no ill will. Although quickly we begin to learn otherwise. My dear Fortunato, you are luckily met. How remarkably well you are looking today. So begins the series of dialogues Song 2 and conversations that would appear to be like any other had by two good friends. At first Montresor is nothing but amicable, he compliments his good friends on his looks, dress and even praises his good taste and reputation. And yet some fools will have it that his taste is a match for your own. The audience now fully engaged in the story though the methods of mystery, curiosity and perhaps even endearment or empathy for Fortunato continue to delve deeper into the event unfolding before our very eyes. Just as the protagonist and the antagonist begin to descend upon the catacombs and the halls of the Montresors manor, so does the conversation and the dialogue taking place between them. We will go back; your health is precious. you are rich, respected, admired , beloved; you are happy, as once I was. You are a man to be missed. For me it is no matter. We are now beginning to be exposed more openly to the sociopathic ways of our protagonist; Montresor. Willingly and cheerfully guiding our poor and still at the moment helpless, Fortunato , as he is still under the spell of Montresor. When we finally arrive to the depths of the catacombs the reader is now aware that some horrible event is bound to ensure, but the conversation and the presentation made by the writer has now fully invaded the reader. Little by little inch by inch as we descended down through the catacombs, we have been made more anxious, more uncomfortable. Now all the built up tension that has been gathering is ripe for the telling. Pass your hand Is the cue that signals the reader. We are here, this is where our journey has been leading you, here is the end. Over the wall; you cannot help feeling the nitre. It is then that Poe unleashes his Single effect all done with a fine attention to detail. The message is clear. you are now here, you will go nowhere, this is where you will rest. Presumably after achieving his single effect the reader will sense a series of emotional responses; ranging from fear to terror to relief. Relief that the built up tension and anxiety has now been released. The rid e has finally come to an end and it is now time to go home and think upon the emotions you have felt here today.

Aspects of Family Law Children are the future and the care which is given to the welfare, education and protection of children is widely recognized as paying dividends in their later years. Children are one of the most vulnerable beings in our society and that is why the protection of the children are one of the most important rights to uphold in society. This assignment will critically discuss areas that uphold these rights such as the constitution regarding articles such as 41 and 42, legislation, various legal and social policies and children in the criminal justice system. This assignment will also include three recommendations about improving the position and welfare of children in the Irish society. CONSTITUTION The first area that upholds children rights is the Irish Constitution, Bunreacht na hEireann. This fundamental legal document sets out how Ireland should be governed and the rights of Irish citizens, (Constitution n.d). In the constitution, Article 41[1] refers to the family. This article gives the family rights, which outweigh any other rights. It is a special protection from the State to grant the ideal environment to raise a child, (Ferguson and Kenny 1995). The State protects and values the family but only as a unit. This unit refers to a married family, which concludes that this protection is only to a married family. The family unit in Ireland has autonomy over and above that of the individual members of the family, (Geoffrey 2003).   Once the parents are not married, the father has no constitutional rights to his child, (Nestor 2004). Article 41 enshrines the protection of the family from undue interference by the State and titled the balance institutionally towards the enchantment of parental rights and the minimum intervention end of the continuum, (Duncan 1993). Article 42[2] states that the very first people to educate a child are the parents. This is a duty that is imposed by the Constitution and in return for this duty, the custody and guardianship are guaranteed to the family. The State guardian of the common good also provides free education for the children. On the other hand, Article 42 is not all just about education. Article 42.5[3] concludes that when parents fail to look after and care correctly after their children, the State will step in and intervene. The precautions that will occur will be the State becoming the parent of the child or find substitute parents. The factors that allows the State to intervene range from child abuse, neglect and very serious cases. However the Constitution still did not define the rights of children as distinct from those of the Family, (Childrens Rights Alliance, Childrens Rights 2012). With this article, children were still seen as a possession or belonging and not an individual with rights. On the 10th November 2012, the people of Ireland held a referendum in order to change to text of Article 42.5. Article 42.5 was deleted and Article 42A was inserted, (Quinn 2012). The legislative perspective of this Act was about the childs best interest and since it was enacted children were granted the same fundamental and unenumerated rights as adults, (Childrens Rights Alliance, Childrens Rights 2012). This Act puts children first and sees a child not just as a belonging. In addition, Article 42A gives the child a choice to make their own decision but the views of the child shall be made certain of balance with regard to age and maturity of child. Before the referendum, there were some serious tragedies, one horrible case, in what most people until this day say, the children involved in this disaster were failed by everyone around them, even Judge Miriam Reynolds (RIP) agreed with this statement[4]. The reason everyone had failed these children was the fact the Western Health Board had been involved since 1996, but the children had not been taken into care until 2004. Article 42A takes into consideration the vulnerable situation of a child, in that they are largely dependent on adults for their care and are often powerless to justify and uphold their own rights. The Constitution was amended so that family and the child can be separated and that the childs life and rights are mirrored to the parents. Furthermore, it was amended to put in place for a more efficient child protection system, (Childrens Rights Alliance, Childrens Rights 2012). On behalf of the child, 42A is a constitutional development for the protection of children and with hope, no more cases such as the Roscommon Child Care Case will happen again. A recommendation about improving the childs safety and welfare in Irish society will start with amending our Constitution. Article 41 does not recognized an unmarried couple with a child as a family, (Nestor 2004). This is a grave problem, as unmarried parents are not considered a family. Article 41 does not protect unmarried couples as it does with married couples. This gives problems with the constitutional rights and there will be a problem to intervene. The father also has no custody or guardianship to this child, if he and the mother are not married, unless he applies for the guardianship. This is very unfair, outdated and should be changed. It should be changed for the childs safety and welfare as it is not the childs concern if their parents are married or unmarried. LEGISLATION. The Child Care Act 1991 is a primary piece of legislation, which safeguards a childs health and safety[5]. It regulates all child protection and it imposes a positive mandatory obligation on the HSE to promote the welfare of a child in its area who are not receiving adequate care and protection (Ireland, Department of Children and Youth Affairs, 2010). In another one there will be a window so that they can see where the sun is. In the third one there will be no windows and no clocks. All they would have is a light bulb in the ceiling. All the booths will have a bed and a bright red button on the far wall. The button will be for ringing. They will ring when they are tired, hungry, or need to use the facilities. My assistants and I will record when they become hungry and when they become tired. They will be in this room for three days. Armed with knowledge about the infection, transmission, environmental factors, alternative treatments, prevention and ways to control C. diff healthcare workers can better be prepared to help fight the spread of this deadly infection. A C. Diff, its transmission and environmental factors C. diff is a gram –positive, anaerobic, spore forming bacteria. It colonizes the intestinal tract of those infected after normal intestinal flora has been disrupted by antibiotic therapy. Diagnosis of C. diff is based on clinical symptoms supported by endoscopic finding or stool testing for the presence of the pathogens or toxins. Patients that has taken antibiotics within the past 3 months or a patient that has diarrhea 72 hours after hospitalization should be tested. ELISA is the test used to diagnosis C. diff. It checks for the toxin A or B or both. C. diff incubation period is 1 to 2 weeks. Asymptomatic carriage can range from severe diarrhea, pseudo membranous colitis, toxic mega colon, intestinal perforation, and death from secondary sepsis. C. diff toxin founded in stool ranges from 1% to 2% in normal population to 10% in hospital inpatients and up to 85% to 90% in patients with proven AAPMC. Some risk factors for C. diff includes being over the age of 65, patients who are immunocompromised, gastrointestinal surgery and procedures, gastrointestinal stimulants, antiperistaltic drugs and proton pump inhibitors. The transmission of C. diff can be transmitted by another patient. The transmission can be via commodes, thermometers, bedside tables, floors, and other objects in rooms used by a patient with C. diff. It can also be transmitted from the hands of healthcare workers. That’s why it’s important for healthcare workers to use proper hand hygiene techqines. Healthcare workers must wash hands with soap and water because antiseptic hand santaniers doesn’t destroy C. diff. Environmental factors also play a huge part in the transmission of C. diff. Healthcare professions should know the proper environmental disinfectants to use. Housekeeping staff should ensure proper cleaning and disinfection of equipment and the environment. Patient’s rooms should be properly cleaned at least once a day with proper equipment for isolation rooms. In order to stop the transmission of C. diff all departments must work together to secure that C. diff transmission isn’t constantly transmitted to other patients. Treatments C. diff will usually clear in 20% to 25% of patients with mild infection and the discontinuation of precipitating antibiotic. When patients have moderate to severe diarrhea or colitis oral metronidazole is a treatment used for 10-14 days at a total of three times a day. Metronidazole achievement rate is usually 95% effective. Vancomycin is also used for 10-14 days at a total of four times a day. Vancomycin sometimes promote emergence of VRE (vancomycin-resistant enterococci). If a patient develops fulminant colitis admittance to ICU is necessary and an emergency colectomy is performed because of severe ileus or impending perforation. Mortality rate is 30% to 80% if surgery is performed. Prevention. Prevention of C. diff starts with avoiding the use of antibiotics that are associated with high rates of C. diff. Healthcare staff, housekeeping, dietary, and maintenance should be informed on the transmission and precautions of C. diff. When C. diff is suspected or verified patient should be place in isolation. The proper precautions should be placed on the door of the resident and visitors should be directed to nursing station before admittance into the room of a person with C. diff. Healthcare workers should drape with gowns and gloves for all contact to prevent the spreading of C. iff. Proper handing washing should be done with hand soap and water because sanitizer with alcohol doesn’t help in the fight against C. diff. Patient care equipment should remain in the room throughout isolation. Patient rooms and equipment should be cleaned with bleach, alkaline gluteraldehyde, or ethylene oxide. Always remember never to mix different solutions because this can cause toxic vapors. All rooms should be terminal cleaned once patient is discharged to prevent further spreading of C. diff. Health Promotion and Wellness Strategy. The Center for Disease Control and Prevention has tool kits for healthcare professional. CDC offers up-to-date information, recommended infection control measures, and tools for outbreak response coordination and reporting. Under our study the passage of the story ‘Doctor in the house’ written by a famous British writer and an assistant editor of the British Medical Journal of the 20 th century Richard Gordon who worked also as a ship’s surgeon, but left his medical practice in 1952. The story ‘Doctor in the house’ is one of Gordon’s twelve ‘Doctor’ books and is noted for witty description of a medical student’s years of professional training. ‘Doctor in the house’ is not really intriguing title for the story, but, perhaps, this simplicity makes this story so attractive for an experienced reader, who is able after looking through passages to find extremely interesting phrases, word combinations, unusual style of writing and etc. Speaking about the style of the story we can say that it is autobiography. The plot of the passage is build round ‘an unpleasant inevitability’, we mean the final examination of medical students. At the beginning we dive in the meaning of the final examination in medical universities. Here we meet the main character who is the young author at the same time. Trying to pass his exams the author managed to pass his true emotions and feelings before, during and after exams. What is more, Gordon gives us a brilliant systematic structure of the social types of people using a little bit irony. Well, when young Richard knew his results, the whole world stood still round him and everything was calm. We see so-called ‘Happy End’ of the passage. In my opinion, in these pages we see a conflict which can be described as the conflict between man and fate. The person who saw many examples during the examinations different styles of behavior, and he was getting nervous about failing his chance. Even his friends told him that ‘One doesn’t fail exams. One comes down, one muffs, one is ploughed, plucked or pipped. These infer a misfortune that is not one’s own fault. To speak of failing is bad taste. It’s the same idea as talking about passing away and going above instead of plain dying. ‘ The general atmosphere of the extract is ironic. We can see it from the first sentence of the passage in a phrase ‘ an unpleasant inevitability to be faced sooner or later’. During the text we see a lot of examples of the irony such as the description of the visiters in the waiting-room, the process of passing exam in Cambridge and some others. This stylistic device was used by Gordon to make his story lighter and funnier for the reader and, perhaps, future and still students of the medical examinations, and maybe not only for them but for all students who prepare themselves to any exam. The vocabulary is full of colloquial words, but at the same time he used special terms, in my opinion it is used to show that as the narrator is medical student, of course, he knew the medicine terms. What makes the author’s language interesting there is a great number of epithets ( an unpleasant inevitability, fragrant cheating, frustrated brilliance, imminent thunderstorm and others), simily ( exam like death, porters were like the policemen that flank the dock at the Old Bailey, candidate will come to the end of his interrogation struggling like a cow in a bog), metaphors ( feeling if I had just finished an eight-round fight, Speaking about syntax I can say that it is quite varied from very simple sentences which are refer to very difficult sentences which transfer all features of oral communication (‘I started off confidentially, reeling out the lines of treatment and feeling much better’). To sum up I want to say that I really like this story because a lot of ideas to think. The lesson from this passage is that you can get everything if you try hard, and you should always believe in good, but to be prepared to the worst, as Russsian proverb said.

Hurley et al (2009) in their study on effectiveness and clinical applicability of integrated rehabilitation programs for knee osteoarthritis showed that correct integrated rehabilitation programmes involving exercise and self-management are more clinically and cost effective and may be the best way of managing the large and increasing number of people suffering chronic knee pain. Another example from Santos et al (2004) in their project implementing clinical evidence in the management of coronary care provided a foundation for the development of a management strategy by using a multidisciplinary team approach, involving updating guidelines and resources. A positive outcome of the project was a reduction in hospital admission. An example in our local services was seen in a study of rehabilitation and management of elderly individuals following stroke which required an integrated approach from a multidisciplinary team. This minimised readmission of chronic cases and proved cost effective. CONCLUSION This essay has discussed the mechanism of clinical effectiveness and evidence based practice in the context of physiotherapy and rehabilitation service. The essay has reflected on the needs and future expectations in the provision of a safe and effective care on a national scale. It has been found that in the context of providing effective rehabilitation, the implementation of up-to-date guidelines, auditing and cost evaluation are all paramount for the assessment of clinical effectiveness. In conclusion, clinical effectiveness in physiotherapy and rehabilitation services uses an integrated approach and can be thought of as the sum of the right person; as being the competent skilled therapist, backed with the right evidence, protocols and guidelines (the right way).
